Use the added toggle to rotate the spike to operate on yourself!
Area of Effect indicator
Make sure you're aiming at a head you can operate on!
Additional note: Effect won't follow the player's head. If the receiving player wants to opt out, all they have to do is walk away.

Heads up
Not officially quest compatible. The shaders used are not quest compatible.
However, some small tweaks will make it operational on quest, just without the eye effects.
